We take the role of a home care provider in Minnesota very close to our hearts. Every effort that we spend is done with compassion for our patients – especially during this pandemic.

The Department of Human Services has provided initiatives in response to this pandemic. One of the programs is the Community Access for Disability Inclusion (CADI) Waiver where home and community-based services are provided to people with disabilities, those who require the level of care in a nursing home. As an alternative to institutionalization, these services enable patients to live independently in community settings.

Eligibility starts with the person choosing to live in the community. They must be eligible for medical assistance (MA) based on the department’s disability status. Applicants should be certified as disabled by the State Medical Review Team or Social Security. They should be no older than 65 years old at the time of application. Assessments such as MnCHOICES and others will be conducted to state the level of care needed and the appropriate supports and services.
